- I hereby authorize Hampton University Men's Lacrosse Program staff to act for me, according to their best judgment, in any medical emergency. As parent/guardian of aforementioned camper, I take full responsibility for payment of injuries that may occur during the Hampton University Men's Lacrosse Summer Camp and I hereby waive and release said persons from any liability of illness/injury incurred while attending camp.
- The law requires that parental permission be obtained for emergency operative procedures on minors. The following consent form should be signed by the parent so that procedures may be promptly carried out, and so that no unnecessary delays will occur with operative procedures. However, no operation will be performed, except emergency, without parents being contacted or fully informed.
- I give permission to administer over the counter medicine as well as such diagnostic, therapeutic, and operative procedures as may be deemed necessary.
- I find my son to be physically fit to attend and participate in the Hampton University Men's Lacrosse Summer Camp, and I know of no impairments which would limit her participation in all activities in camp.

NCAA Rules and Regulations
Institutional staff members or representatives of its athletics interest shall not employ or give free or reduced admission privileges to a high school, preparatory school or two-year college athletics award winner.
"Senior prospective student athletes" shall not be permitted to enroll, participate or be employed on an institution's summer sports camp. The definition of a senior prospective student athlete is "one who is available for admission to a member institution or has started his senior year in high school".
The Hampton University Lacrosse Summer Camp is open to any and all with entrance limited only by number and age.
